At Charlotte's instigation, Iris abuses her medical skills to make Emma believe she's loosing her mind, like her ma at the same age. Emma ends up signing over her proxy to Iris, who now dumps Roland. Seeing his romantic dream shattered, he tells Emma about Iris's betrayal, including Marco being the unborn twin's biological father.
