After a night together - one that neither Dr. Nice nor Charlie can quite recall - nothing is the same. A mysterious wave of illness at a Flensburg shipyard is keeping Dr. Nice and the hospital staff busy. Several workers are suffering from high fever, shortness of breath, and neurological deficits, yet neither lab results nor initial examinations provide an explanation. As the situation escalates, Nice follows his intuition and seeks the source of the problem directly on-site. At the same time, his private life takes an unexpected turn: after finally moving out of his hotel, he ends up - of all places - in the guest room of his arch-rival, Dr. Schmidtke. Caught between an involuntary living arrangement and family tensions, Nice must once again prove that unconventional methods can sometimes lead to success. When a surprising lead from overseas finally sheds light on the mystery, it becomes clear that the solution lies closer to home than anyone had suspected.

After a serious workplace accident, ferry mechanic Heike Kärkering is admitted to the hospital. Although the initial surgery goes well, her condition rapidly deteriorates. A mysterious complication with wound healing threatens her leg, posing a difficult medical challenge for Dr. Nice and the hospital team. It takes an unexpected clue to point the way toward the correct diagnosis. While Dr. Nice does everything in his power to save his patient, his personal life is also in flux: after checking out of the hotel, he moves onto a houseboat, attempting to make a fresh start. However, the search for a home, tensions with Lea and Charlie, and unresolved issues from his past prevent him from finding peace. Amidst family misunderstandings and a race against time, Dr. Nice comes to realize that not all wounds are visible - and that some can only heal once they are confronted.

No sooner has Dr. Nice settled back into the *Ostseeschlösschen* than he becomes involved in an unusual case: Marvin, a young musician, is admitted following an accident with a strikingly high blood-alcohol level. For his mother, naval officer Miriam Nicolai, the situation seems clear-cut. At the same time, Nice is forced to confront his own fears: an innovative research program offers the first glimmer of hope that he might regain the use of his injured hand—but this also opens the door to a return to a life he thought he had long since left behind.

The fall from the ladder initially seems harmless. But shortly afterwards, Dr. Schmidtke begins behaving in a completely uncharacteristic way: the usually composed chief physician becomes pensive and laid-back, increasingly losing interest in the life he has led until now. When he even starts questioning his career and vanishes without a trace, Dr. Nice begins a frantic search for the cause. While all medical tests come back clear, Nice grows increasingly worried about his friend. The trail eventually leads far back into Schmidtke’s past - to a time spent in Central America. It takes a crucial clue to finally put Nice on the right track. At the same time, Nice faces a major decision of his own. Progress in his hand therapy opens new possibilities but demands courage and determination. In his personal life, too, he must confront uncomfortable truths: Lea demands more reliability from her father, Charlie struggles with feelings of loneliness, and old relationships cast long shadows.

Expectations are high, and international attention is focused on the event. Yet, shortly before the surgery, everything is thrown into jeopardy.
