Yejin rejects Do Ji's marriage proposal. Hur Jun encounters a mysterious man and begs to be taken in as his apprentice. Back at the infirmary, Hur Jun gets demoted. Yoo Doji confesses his love for Yejin to his parents.

By day, Hur Jun works his job at the infirmary. By night, he diligently studies under his new mentor. Dr. Yoo takes Hur Jun to treat a bedridden patient. Mrs. Yoo eagerly plans to marry off both Yoo Doji and Lady Yejin.

Dol Swe suddenly shows up at the infirmary demanding Hur Jun to heal his mother's blindness. Meanwhile, Yoo Do Ji is given his first challenge in the palace.

Dr. Yoo recognizes Hur Jun's ability and puts him in charge of the medicine warehouse. Dahee gives birth to a baby boy. Busampo becomes disdainful. Dahee learns about the hardships of being lower class.

Hur Jun concedes that he cannot provide for his family while working in the infirmary. Just as he is on his way to tell Dr. Yoo that he will quit, he and Lady Yejin are ordered to treat a noble lady suffering from paralysis.

After being punished for not curing his wife, former Deputy Prime Minister gives Hur Jun another chance. Yoo Doji is brought in, though only just in time to witness Hur Jun receive praise.

Hur Jun returns home with many gifts from former Deputy Prime Minister. Mrs. Yoo catches wind of this and demands they be forfeit to the hospital. Dr. Yoo expresses his disappointment when he thinks Hur Jun is trying to take the easy road.

Hur Jun's friends and family try to encourage him to stop being depressed. Koo Il Suh's wife becomes pregnant. Yoo Doji treats patients. Busampo makes Hur Jun a proposition.

Hur Jun quickly regrets his partnership with Busampo. Still too ashamed, he goes to work in the Iron Mines. After a mine explosion, his doctoral instincts return.

After coming to the realization that Dr. Yoo cannot be swayed, Hur Jun decides to go to Samjuk Temple to continue learning medicine under a new teacher, Kim Min Se, the Priest. Dr. Ahn tells Hur Jun about Kim Min Se's past.

Dr. Ahn recounts the tragedy behind Dr. Kim Min Se's son. Hur Jun cares for the lepers in Samjuk Temple. In order to prevent Lady Yejin from becoming a distraction to Yoo Doji, Mrs. Yoo sends her away. Sang Hwa and his lover elope.

Hur Jun rescues Sang Hwa and returns to Samjuk Temple. Impatient with his progress in treating leprosy, Dr. Kim Min Se tries an unorthodox medication on himself. Yoo Doji struggles with his feelings.

Yoo Doji and Hur Jun both begin their respective journeys on route to take the medical examination.

Even though he is in a hurry to take the medical examination, Hur Jun reluctantly sees local patients. Lady Yejin returns to Dr. Yoo's infirmary.

It's a race against the clock, as Hur Jun only has two days to travel 100km to Seoul in order to take the medical examination. Yoo Doji retakes the exam.

Celebration ensues for Yoo Doji's passing of the medical examination. Rumors of Hur Jun's righteous deeds precede his eventful return to Sanum.

Hur Jun learns not all ailments can be cured. Dr. Yoo's condition worsens. O Geun struggles as the only doctor remaining at the infirmary. Yoo Do Ji receives a promotion.

Lady Ye Jin arrives in Seoul to search for Yoo Do Ji. Dr. Yoo regains consciousness due to Hur Jun's efforts. Yoo Do Ji gets married. Word spreads of Dr. Yoo's imminent demise.

Dr. Yoo bids farewell. Hur Jun performs Dr. Yoo's final lesson. Dr. Ahn fulfills his desire of human dissection. Hur Jun goes to Seoul to inform the Yoos of Dr. Yoo's passing.

Mrs. Yoo retrieves her remaining possessions from the hospital and puts it up for sale. The remaining hospital staff go their separate ways. Yoo Do Ji shows his greed when the Queen becomes pregnant. Hur Jun studies for the medical exam.

Both Hur Jun and Oh Geum advance to the second part of the examination. Hur Jun's family relocates to Seoul. Yoo Do Ji gets a promotion. Da Hee struggles in her new environment.

Da Hee has a miscarriage making Hur Jun late for his first day at work. Yoo Do Ji attempts to make Hur Jun's palace life difficult. Koo Il Suh hatches a plan to move to Seoul. Lady Ye Jin joins the medical staff.

Hur Jun is curious about the civil medical center. The Koos realize that Seoul is not what they anticipated. Lady Ye Jin struggles with her additional responsibilities of palace nurse. Hur Jun enrages his superiors by being insubordinate.

Hur Jun's placement at the civil hospital is decided, to the surprise of his colleagues. Oh Geum sets up a meeting between Hur Jun and Lady Ye Jin. Mrs. Koo sows the seeds of suspicion in Da Hee's mind.

Lady Ye Jin and Chae Sun are accused of stealing from the warehouse. Yoo Do Ji becomes distressed when he learns that Lady Ye Jin has been working as a nurse. Hur Jun becomes enraged upon learning that the civil hospital extorts patients.

Hur Jun's palace enemies quickly come back to haunt him as he is punished for having a private medical practice. Lady Ye Jin nurses Hur Jun back to health. The civil hospital is overwhelmed with sick children. The Queen goes into labor.

Yoo Do Ji seeks civil hospital nurse Hong Choon's advice on dealing with the Queen's birth complications. Hur Jun gets infected while treating a civil hospital guard who has become afflicted with impetigo. Lee Jung Myung courts Lady Ye Jin.

One night, a medical keeper hangs himself in the herb storage. Yang Tae, Koo Il Suh, and Oh Geum blunder a deal. Hur Jun gives a training course on acupuncture to the nurses. Lee Jung Myung's acquaintance suddenly dies mysteriously.

Hur Jun informs Lee Jung Myung that his associate, Mr. Ku, was poisoned over a long period of time. Hur Jun hatches a plan to find out from where the poison originated. Da Hee is discovered by her brother.

Lee Jung Myung is arrested for treason under politically motivated pretenses. A nurse attempts to seduce Yoo Do Ji. The new deputy minister, a familiar face from the past, is introduced. The Queen's brother is suffering from Bell's palsy.

The new deputy minister suspects something is awry when Hur Jun heals a Bell's palsy patient in three days, whereas Yang Ye Soo and Yoo Do Ji said it would take seven days.

Hur Jun continues to treat the Queen's brother. Yang Ye Soo and Yoo Do Ji are anxious that Hur Jun will reveal that the Queen's brother has stomach cancer. Da Hee and Mrs. Koo try to woo their husbands. Hur Jun makes the king a promise.

As the Queen's brother's illness seems to take a turn for the worse, Hur Jun's rivals all take solace in the fact he will receive punishment.

Hur Jun's family is showered with various gifts from the King. Royal physician Yang Ye Soo confides in Hur Jun. Yoo Do Ji gets blackmailed by a nurse.

Hur Jun gets promoted to 7th rank, while Yoo Do Ji is relegated to delivering messages to China, partly because of the rumor of his affair with a nurse. Oh Geun suffers from lovesickness. Mrs. Yoo takes her anger out on Da Hee and Hur Jun.

After adamantly refusing, Mrs. Yoo reluctantly agrees to be treated by Hur Jun. Chae Sun extorts a local. Da Hee makes peace with Mrs. Yoo. Lady Ye Jin is among the nurses selected to perform bedside services for foreign ambassadors.

Lady Ye Jin and Sook Jung are selected to give sexual pleasures to the Chinese ambassadors. Hur Jun accompanies the King on a hunting excursion. While there he is reacquainted with an official who knows about his dark past as a smuggler.

The police general who recognizes Hur Jun starts investigating him. Hong Choon reciprocates Oh Geun's gestures. Hur Jun is dispatched to the countryside where plague is raging throughout.

Just when the doctors think they found a cure to the plague, it relapses. Chae Sun contracts the plague. Yoo Do Ji returns from China. The police general reaffirms that he will arrest Hur Jun after a cure for the plague is found.

In recognition for his work on finding a cure for the plague, Hur Jun gets promoted to 6th rank. Hur Jun's rivals catch wind of his tainted past and inform their superiors.

Hur Jun and Da Hee are arrested. The King demands Hur Jun to be exonerated of all his crimes. When the Queen falls ill with a myocardial infarction, the King sends for Hur Jun hoping he can revive her.

The palace officials are in an uproar as the King calls in Hur Jun to cure Gongbin Queen. Hur Jun and Da Hee are released from prison. Oh Geun worries about Hong Choon's condition. Inbin Queen becomes pregnant.

Hur Jun and Yoo Do Ji both get promoted to royal physicians to look after each queen. Oh Geun proposes to Hong Choon. Gongbin Queen becomes depressed after losing the King's affection. Hur Jun takes a sabbatical.

Ten years later, Gongbin Queen's son Prince Kwanghae confides in Hur Jun. Hur Jun's son, Gyum, states that he does not wish to follow in his father's footsteps. Queen Inbin's son, Prince Shin Sung has been suffering from a skin disease.

Yoo Do Ji's scheme to get Hur Jun kicked out of the palace backfires as Hur Jun is promoted to 3rd rank. Shortly thereafter, the Japanese invade. Hur Jun attempts to salvage the medical books from the library before it's destroyed.

A mob sets the palace and hospital alight. Hur Jun rushes into the burning building after hearing Lady Ye Jin is still inside. The King and his entourage flee Seoul as the Japanese invasion closes in. Hur Jun gets separated from his family.

Sang Hwa sacrifices himself. Yoo Wol gives birth. The King's advisers urge him to escape to China. Even though Hur Jun successfully makes it to Pyongyang castle, the King feels betrayed. Yoo Do Ji looks after the King's health.

At Pyongyang castle, the Prince fights back the Japanese army. In Weeju, Inbin Queen's son faints. Hur Jun is ordered to go to Weeju to look after the gravely ill King. Gyum finally understands his father. Yoo Do Ji is thrown into prison.

Ten years later, Inbin Queen gives birth to a son. Both political factors attempt to court Hur Jun. The palace doctors struggle to compile the finishing touches to their medical textbook. The prince is diagnosed with malaria.

Both political factions are confused after Hur Jun prescribes the prince with arsenic as medicine for his malaria. Hur Jun gets concerned that the doctors will not be able to complete the textbook. The King falls severely ill.

As the King's death imminently approaches, some court officials try to persuade Hur Jun to keep silent about King's written declaration to enthrone Prince Gwanghae as king. Hur Jun is exiled.

Even in exile, countless patients go to Hur Jun to seek medical care. The medical text Dongui Bogam is completed. Hur Jun retires to the old hospital in Sanum to treat poor patients, eventually becoming sick himself.
